Please contact the office to discuss your particular circumstances as multiple insurance companies can be directly billed or require reimbursement.
Please note the cancellation policy. Cancellations within 24-48 hours of your scheduled appointment or no show will be subjected to a 50-100% cancellation fee depending on the appointment type.
